IV therapy, what is actually in the bag
A clinician walks through the ingredients in common IV drips, what each one does, and which protocols have evidence behind them.

IV therapy went mainstream over the last decade. You can now get a drip in a hotel, a gym, a med spa, or a primary care office. The quality varies wildly because the ingredients vary wildly. This paper opens the bag and walks through what is in there.
The base fluid
Every IV starts with a base, usually one of three:
- Normal saline (0.9 percent sodium chloride). The most common base. Matches blood salinity, hydrates fast.
- Lactated Ringer’s. Adds potassium, calcium, and a buffer. Often used after intense exertion or significant fluid loss.
- Dextrose 5 percent in water. Used less often for wellness drips. Adds a small amount of sugar.
For a healthy adult coming in dehydrated, 500 to 1,000 mL of normal saline restores fluid volume in about 30 to 45 minutes. That alone often accounts for the “I feel better” effect more than any vitamin added afterward.
B-complex vitamins
The most common additive is a B-complex blend. The B vitamins are water-soluble cofactors for energy metabolism. They include B1 (thiamine), B2 (riboflavin), B3 (niacinamide), B5 (pantothenic acid), and B6 (pyridoxine). They are safe at the doses used clinically because excess is excreted in urine.
B-complex shows up in almost every drip because the price is low and the safety margin is high. The catch: if you eat a normal diet and you do not have a specific deficiency, the clinical effect is modest. The exception is patients with documented malabsorption, recent gastric surgery, alcohol use disorder, or a known restrictive diet.
Vitamin C
Ascorbic acid at 1 to 10 grams is a frequent addition. High-dose IV vitamin C has been studied in oncology supportive care and septic shock, with mixed results (NEJM, 2018; LOVIT trial 2022). For wellness use, doses of 1 to 5 grams are generally well tolerated. Patients with a G6PD deficiency should not receive high-dose vitamin C. We screen for this when relevant.
Glutathione
Glutathione is a tripeptide antioxidant. The body makes it. IV glutathione bypasses gut absorption and raises plasma levels briefly. It is most commonly used in liver detox pathways, oxidative stress conditions, and as part of recovery protocols. The clinical data is limited but the safety profile in healthy adults is reasonable at standard doses.
Magnesium
Magnesium sulfate is added to “calm” drips and to recovery drips. It can reduce muscle cramping and supports nerve and cardiac function. IV magnesium is faster acting than oral because oral magnesium is poorly absorbed (around 30 to 40 percent bioavailability). Patients with kidney impairment need a cautious dose.
B12
Cyanocobalamin or hydroxocobalamin (the two common forms). Some patients absolutely benefit from B12, especially those over 50, vegetarians, those on metformin or proton pump inhibitors, and anyone with a history of pernicious anemia or gastric surgery. We have a separate paper on whether injections beat pills, but in short, for documented deficiency the IV or intramuscular route works and works fast.
Amino acids
Some recovery drips include taurine, arginine, citrulline, glutamine, and lysine. The rationale is muscle recovery and circulation support. The evidence for IV delivery beating oral intake in healthy adults is thin. If you eat enough protein, you probably do not need them in a bag.
What we do not put in drips
We do not run drips that include compounds without a clear indication and a clean safety profile. We will not chase trending ingredients without data. We also will not run a high-dose drip without confirming a few labs first, because the wrong drip in the wrong patient can cause real harm. A patient with undiagnosed congestive heart failure does not belong in a one-liter saline drip. A patient with iron overload does not belong in a high-dose vitamin C drip.
Who actually benefits
Honest version: IV therapy helps most when there is a real underlying issue. Common situations where the response is meaningful:
- Acute dehydration from gastroenteritis, hangover, or heat exposure.
- Documented B12 or iron deficiency that has not responded to oral supplementation.
- Migraine protocols that include magnesium, an antiemetic, and fluid.
- Post-illness recovery where appetite has not returned yet.
For a generally healthy person who eats well, sleeps enough, and stays hydrated, a wellness drip is a pleasant 45 minutes. It is unlikely to transform anything.
